Inselspital consists of the following directorates: Chief Executive Officer, Medical Directorate, Teaching and Research Directorate, Nursing Care/MTT Directorate, Services Directorate, Operations Directorate and Human Resources Directorate. They have been given decision-making authority by the Board of Management in their respective field with regard to the other directorates as well as the divisions (or their individual departments and institutes). 

The CEO's Directorate directs the strategic positioning process of Inselspital as well as the development and implementation of management tools, strategic planning, strategic controlling, risk management and the continuing development of the organisation of the hospital as a whole. Inselspital Legal Service provides support in legal questions; the Office for Communication and Media takes care of both internal and external communication.

The CEO is Chairman of the Hospital Management Board and Extended Hospital Management Board.

The Medical Directorate coordinates the medical concerns of Inselspital's units and departments. Heads of departments and institutes are directly answerable to the Medical Director.
